10/20/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. She indicated to QMHPA her morning was going well. Intervention: QMHPA inquired about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA continued to encourage Ms. Broadnax to identify her mood and demeanor. QMHPA suggest using her journal to monitor her depressive symptoms for the next two weeks. QMHPA supports/suggests ways she can restructure her time to where she is motivated make changes to her negative behaviors. QMHPA encouraged Ms. Broadnax to become accountable for her actions and reactions as well as being responsible for herself through developing some skills in independence to lessen her dependency on others. QMHPA continued to reinforce the importance of improving her mental health by taking her medication as prescribed and attending all of her scheduled appointments as well as improving her ADLs/IADLs such as learning to manage her finances, complete chores around the home, take her medication as prescribed, incorporate personal hygiene and grooming into her daily routine. QMHPA and Ms. Broadnax review ISP goals. QMHPA and Ms. Broadnax review each of her medications. QMHPA encourages the Ms. Broadnax to begin journaling. Response: Ms. Broadnax appeared to be receptive to QMHPAâs services and interventions that were provided to her during the session. Ms. Broadnax shared that she is working towards building those poor relationships, she shared that she continues to have difficulty trusting her family. Ms. Broadnax has expressed in many sessions that she wants to rebuild and strengthen her relationship with her mother, but she has not communicated with her since the incident over the holidays. Ms. Broadnax remained receptive to improving her communication, socials involvement in her community and decision -making skills. Ms. Broadnax admits that she has made a lot of poor choices and decisions, but only feels that her actions negatively impact her and not others. Ms. Broadnax also informed QMHPA that she has difficulty remembering thing due to her stressors. Ms. Broadnax remained receptive to improving her mental health and her mood swings. Ms. Broadnax admits that she has been compliant with taking her medication because she knows she needs to take them daily. Ms. Broadnax informed QMHPA that she will continue to meet with her psychiatrist to assist her with stabilizing and helping to improve her mental health. Ms. Broadnax was receptive to improving her ADLs/IADLs. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
